The reason why it looks red and swollen is b/c they took it right after it was done, which is what all tattoo artists do to showcase their work either on their shops website or in their portfolios. Getting a tat IS painful since its a vibrating needle carving into your skin, it just depends on how much pain you can handle and for how long.
I have one in the same spot, which didn’t hurt that badly and didn’t bleed all that much either. If I were him, I probably would of put it somewhere different, with different lettering.
